Transmission shifting/Idle Speed problems

My 1991 Integra ls with automatic transmission that I bought has some problems.

First of all, when I crank it up and put it in drive the rpms will drop down about 300 rpm and the steering wheel and dash will start vibrating like there's an earthquake. Then, I take off and the rpm jump up very quickly as I'm speeding up. As I drive it down the road, it stays in first gear for way too long and will stay until I get up to about 3500 rpm or 4000 rpm and let off the gas, then it sort of jumps up another 200 rpm for a split second and shifts to second gear. All shifting after first gear are fine. This only happens when the car is hot. When I run it cold first thing in the morning, it shifts just fine. Any suggestions? I really do not want to rebuild the thing. Any comments would be greatly appreciated.
 
have you checked your transmission fluid?.. probably needs to be changed, make sure you drain and fill it

I changed the trans fluid about a week ago and it didn't help much. It shifts a little better but it still takes a lot to get it to shift into 2nd gear. I also tried to adjust the throttle cable to try and get the idle up a little bit, but the idle speed still jumps around when it's in drive. Sometimes it's high and sometimes it's low.

Thankk you for the reply. I will try to figure out whether this is causing the problem or not. Would it possibly be dirty? Or would I have to replace it?
